disclaimer in book

Hi all ... I'm writing to ask about a disclaimer in an upcoming book to be published ... the book is being self-published and is about addiction/recovery.  The book is based on true events of the writer; names in the book have been changed with the exception of author's brother who has agreed to have his name in the book since the author and his brother who are twins have an organization together ... www.rockstarsuperstarproject.com

Thanks to some feedback from a previous discussion I'm thinking about this as a disclaimer:

These stories are based on true events of the author's experiences, although some names have been changed for privacy reasons.  The author's family members names are correct, thanks to them kindly giving permission for their usage.

This is something we have written to go on the copyright page:

NOTE TO THE READER:  This book is sold with the understanding that the publisher is not engaged in rendering psychological, financial, legal, or other professional services.  If expert assistance or counseling is needed, the services of a competent professional should be sought.  Any application of the material in the following pages is at the reader’s discretion and sole responsibility.

So my questions are:

1) Thoughts about the disclaimer ... would you change anything?

2) Should the disclaimer go in with the preface?

My concern is making sure that we've covered what we need to ... Any and all feedback is much appreciated!
