I would recommend Susie at www.sgwdesign.com .  She can create a cover from your vision at a reasonable cost.  She only works with referrals so tell her Craig sent you.  If you want original art also checkout www.deviantart.com that cost about $500.  I am using a combination of both for my book, original art done and then the cover created from that; but that does end up being more expensive.  At all depends on what type book you're doing as to the type cover typically used.
